We are so excited that your child will be joining us for VBS!
Some things you may need to know to keep you informed!
1. Snacks are provided every night, please be sure to fill out the allergies section if your child has any.
2. We are starting the party so we will have balloons, disco lights, party lights, etc, please let us know if your child has any sensitivity to lights.
3. Wednesday will be an all out party night, including glow sticks, water slide for elementary, and water tables/sprinklers for preschool. Come with your swimsuits on and covered. Bring a change of clothes! If there are any concerns please let us know.
Theme Nights:
Sunday- VBS Shirts or Rock N Roll Night
Monday- Neon Night
Tuesday- Wear your PJ's Night
Wednesday- Crazy Hat and Water Night
Mission:
We will be taking up new PJ's and Books to provide to the Baptist Children's Home.
Don't have time to pick up those PJ's and Books, we will be taking up an offering every
night to go buy PJ's and Books!
Sunday, August 4th, we will keep the Party Going for our worship service at 10:30.
We want to invite all the kids back to perform, on stage, one of our VBS songs.
Please list the name and information of the primary emergency contact and pick up person in the first section and add an additional pickup person/contact below.
You will need to complete one registration form for each participant you're registering. We apologize for any inconvenience this might present.
Functions and Activities: It is my understanding that participating in the programs and recreational and other activities of First Baptist Church Welcome is a privilege. Prior to my child/youth's participation in such activities, I acknowledge that there are certain risks associated with the activities including, by way of example, physical injury due to activity related accidents, physical injury due to transportation related accidents, illness, or even death. In addition, I acknowledge that there may be other risks inherent in these activities of which I may not be presently aware.
Release of Liability: By signing this Permission/Waiver Form, I expressly warrant that the child/children named above is/are capable of withstanding the physical demands of the activities discussed above. I also expressly assume all risks of the child or me participating in the activities, whether such risks are known or unknown to me at this time. I further agree to indemnify and hold harmless First Baptist Church Welcome and its ministers, leaders, employees, volunteers, or agents from any and all claims arising from my participation in its activities and programs, or as a result of injury or illness of my child during such activities.
First Aid and Emergency Medical Treatment: I recognize that there may be occasions where the child(ren) named above may be in need of first aid or medical treatment as a result of an accident, illness, or other health condition or injury. I do hereby give permission for agents of First Baptist Church Welcome to seek and secure any medical attention or treatment for the child named above, including hospitalization, if in the agent's opinion such need arises. I agree to pay all fees and costs arising from this action to obtain medical treatment. I give permission for attending physicians(s) and other medical personnel to administer any needed medical treatment, including surgery and, again, I agree to pay for the medical treatment.
On occasion, First Baptist Church Welcome takes photographs or makes an audio or video tape recording of children and/or adults involved in church activities. Such photographs or video records may be used by staff and participants to remember the activities or participants. In addition, such photographs and audio/visual recordings may be used in First Baptist Church Welcome publications or advertising materials to let others know about our ministry. I consent to the use of any such audio or visual record of the child/children named above to be used, distributed, or displayed as agents of the church see fit. This consent includes but is not limited to: photographs, videotape, and audio recordings. I understand that there will be no payment for me or my child’s participation in this release. I understand that it is my responsibility to update this form in the event that I no longer wish to authorize the above uses.
A parent or legal guardian must sign this agreement on behalf of any minor participant(s) under the age of 18. By electronically signing this agreement, I represent that I have the authority to sign and enter into this agreement on behalf of the minor(s) listed above.
